    I have read carefully the syllabi, ordinances/statutes and conditions etc. laid down in the prospectus concerning my admission to the course and affirm to abide by the same. I shall also accept any changes/amendments made thereafter from time to time by the University. I shall be responsible for any wrong statement/information recorded in this form and would accept the decision taken by the university regarding the admission as a consequent to any wrong statement/information.
